Former Foster Youth
Former foster youth are youth that have emancipated or aged out of the foster care system. Emancipated youth often need additional services after foster care due to lack of family or support system.
Eligibility Requirements
Ages 18-21
Emancipated (aged out) from foster care

You can obtain a copy from the Licking County Health Department.

You can obtain a social security card from the local Social Security office at 125 Derby Downs Road, Newark, OH 43055 or by calling 866-737-3514 or Social Security office online.
